Around 56% to 94% of the population is stung at least once in theirlifetime by a member of the order Hymenoptera which includes wasps, bees, and ants. The response to a waspsting may vary from mild local reaction to severe systemic and anaphylactic reactions. The clinical picture andmortality rate tend to be more severe in adults compared to children. We present a 32-year-old agricultural workerwho was bitten by multiple wasps while on a coconut tree. In spite of the heavy load of venom due to the multiplebites, the patient did not develop anaphylaxis. However, a delayed reaction did occur within 48 h in the form ofsevere multi-organ dysfunction. There was significant improvement by around 2 weeks; but it took another 6 monthsfor the serum creatinine to normalize. This case highlights the occupational risk of Hymenoptera envenomation, thelife-threatening complications that may follow and which may even be delayed as was the case with this patient, andthe value of emergency care and intensive management which can result in a favorable clinical outcome.

Case Report Acute Disseminated Melioidosis Presenting with Septic Arthritis and Diffuse Pulmonary Consolidation in an Otherwise Healthy Adult: A Case ReportHai Sherng Lee,1 Abdul Azeez Ahamed Riyaaz,1 Seng Hong Yeoh.1 AbstractBackground: Melioidosis is an infectious disease caused by Burkholderia pseudomallei. It is most prevalent in South-East Asia, northern Aus-tralia, and the Indian subcontinent. Septic arthritis is a rare manifestation of melioidosis. Melioidosis is usually found in patients with diabe-tes, heavy alcohol use, or chronic lung disease. Case: We report a case of melioidosis in an otherwise healthy 44-year-old male, who presented with acute painful left knee swelling, high-grade fever associated with chills, rigors and night sweats, and a productive cough. Examination revealed active synovitis with effusion involving his left knee, ankle and elbow joints and scattered crackles over both lung fields. Chest X-ray showed diffuse pulmonary consolidation. Abdominal ultrasound showed splenic micro-abscesses. The diagnosis was made based on a positive blood culture for Burkholderia pseudomallei. He was started on appropriate antibiotics and responded well, becoming afebrile after 48 hours, while his joint effusions disappeared after one week. Conclusion: Septic arthritis only occurs in 4% of patients with melioidosis. When there is diffuse pulmonary involvement, melioidosis may mimic disseminated tuberculosis, sepsis syndromes, and systemic vasculitis syndromes. This case is relevant for medical literature as melioidosis is emerging and is expanding its territories worldwide. It should be considered early in the differential diagnoses in endemic areas so that treatment can be started early to reduce its high mortality and morbidity.

Kollef, MD, FCCP Objective: To assess the effect of baseline variables, including treatment, on outcome in patientswith nosocomial pneumonia due to methicillin-resistant Staphylococcus aureus (MRSA).Design: Retrospective analysis of data from two prospective, randomized, double-blind studies.Setting: Multinational study with 134 sites.Patients: A total of 1,019 patients with suspected Gram-positive nosocomial pneumonia, including339 patients with documented S aureus pneumonia (S aureus subset) and 160 patients withdocumented MRSA pneumonia (MRSA subset).Interventions: Linezolid, 600 mg, or vancomycin, 1 g, q12h for 7 to 21 days, each with aztreonam.Measurements and results: Outcome was measured by survival and clinical cure rates (assessed 12to 28 days after the end of therapy). Logistic regression analysis was used to determine the effectof treatment and other baseline variables on outcome. Kaplan-Meier survival rates for linezolidvs vancomycin were 80.0% (60 of 75 patients) vs 63.5% (54 of 85 patients) for the MRSA subset(p ⴝ 0.03). Logistic regression analysis confirmed that the survival difference favoring linezolidremained significant after adjusting for baseline variables (odds ratio [OR], 2.2; 95% confidenceinterval [CI], 1.0 to 4.8; p ⴝ 0.05). Other baseline variables associated with significantly highersurvival rates in MRSA pneumonia were serum creatinine levels less than or equal to two timesthe upper limit of normal and absence of cardiac comorbidities. Clinical cure rates for linezolidvs vancomycin (excluding indeterminate or missing outcomes) were 59.0% (36 of 61 patients) vs35.5% (22 of 62 patients) for the MRSA subset (p < 0.01). Logistic regression analysis confirmedthat the difference favoring linezolid remained significant after adjusting for baseline variables(OR, 3.3; 95% CI, 1.3 to 8.3; p ⴝ 0.01). Other baseline variables associated with significantlyhigher clinical cure rates in MRSA pneumonia were single-lobe pneumonia, absence of ventila-tor-associated pneumonia, and absence of oncologic and renal comorbidities.Conclusions: In this retrospective analysis, initial therapy with linezolid was associated withsignificantly better survival and clinical cure rates than was vancomycin in patients withnosocomial pneumonia due to MRSA.

Research HospitalDepartment of Medicine (Department of Hematology-Oncology) Our department has been challenging to cure a variety of fatalhematological disorders with currently available methods. Themainstay to achieve our goal is, nowadays, hematopoietic stemcells (HSC) including bone marrow, peripheral blood and um-bilical cord blood cells, transplantation and cytokine therapy.These projects have been under way at the HSCT and Hematol-ogy wards with the excellent assistance of nurses andcomedical staffs. We annually take care of more than 40 pa-tients with autologous or allogeneic HSCT. In an attempt toexpand therapeutic benefits of HSCT, we are actively participat-ing to the nation-wide project of unrelated bone marrowtransplantation as the largest HSCT center in Japan. In 1998,we furthermore have established Tokyo cord blood bank ser-vices in our hospital. Cord blood cell transplantation hasbecome strong weapon for patients sufferred from hematolog-ical malignancy without any related or unrelated stem celldonors. We have already treated more than 25 adult patientsuntil the end of March, 2001. Also since 1998, we had startedclinical gene therapy for stage IV renal cancer patients as col-laboration with other clinical departments in our hospital aswell as other hospitals including Juntendo University Hospital,Tsukuba University Hospital, and National Cancer Center.

The 27th Annual Convention of the IACR named IACRCON –2008 was held at the GCRI, Ahmedabad between the 7th and 9th February, 2008. It coincided with the celebrations of the silver jubilee of the Department of Cancer Biology of the GCRI and the birth centenary year of Dr. T B Patel, the Founder Director whose guidance and efforts have helped establish the Institute to its present stature. The Convention was spaced out between the first and the third day with a total of eight sessions covering various areas of Cancer research, basic and translational, with a global representation. The International Symposium on Frontiers in Functional Genomics between these two days was the high point of the Convention and featured talks on approaches and clinical trials applying latest technologies, to study, understand and treat Cancer.
