Polymeric Micelles – The Future of Oral Drug Delivery Department of Chemical and Biomolecular Engineering University of Notre Dame, Notre Dame, IN 46556 Abstract This work examines current advancements in polymeric micelles as a method for oral delivery of poorly water-soluble drugs. The oral route presents several barriers to drug delivery that the chosen vesicle must overcome. Polymeric micelles have several physical properties, including molecular weight and copolymer block composition, which can be tailored to alter the vesicle structure and overcome these barriers. Examination of current research demonstrates the ability of polymeric micelles to respond to external stimuli, such as pH, allowing for controlled release of encapsulated drugs in the gastrointestinal tract. Lastly, with patients preferring the oral drug delivery route to the intravenous delivery route, it was shown that polymeric micelles can achieve the same desired pharmacological dose via either delivery method. These factors make polymeric micelles appear to be a viable option for future oral drug delivery applications. 1. Introduction 1.1 Clinical Relevance

Rabinovici, M.D The following is intended to be a review of medications commonly prescribed for the management of dementia and associated psychiatric behavioral complications. At this time, no medications are approved by the FDA for the treatment of psychiatric behavioral conditions related to a dementia; however, one second generation antipsychotic, risperidone, is approved in the United Kingdom for treating aggressive/assaultive behavior due to Alzheimer's disease. Hopefully, this will serve as a guide to caregivers and healthcare providers in the safe and appropriate use of medications for these indications. Dementia is a syndrome, or a collection of symptoms, characterized by a progressive loss of cognitive abilities that interferes with an individual's ability to function at work or in their usual personal activities. Dementia can affect memory, thinking, language, visual perception, judgment, and behavior. In early stages, an individual may be able to fully compensate for cognitive problems and continue to function independently. A condition known as "mild cognitive impairment" (MCI) may, in some individuals, result ultimately in a diagnosis of AD. In the final stages, dementia prevents an individual from taking care of themselves or providing for their basic needs. Dementia can be caused by a variety of brain diseases and medical illnesses. An accurate diagnosis of the underlying cause of dementia is the first step in selecting optimal drug therapy. The major causes of dementia are Alzheimer's disease (AD), vascular dementia, mixed dementia (vascular and Alzheimer's combined), frontotemporal dementia (FTD) and Lewy-body dementia (LBD). There are other recognized types of dementia, such as those due to human immunodeficiency virus (HIV), Parkinson's disease, Huntington's disease, major organ dysfunction (e.g. heart, lung, liver or kidney failure) and substance abuse, but these are beyond the scope of this article. The following suggestions and comments will focus primarily on AD, which is the leading cause of dementia in the United States and Europe. But, it's not what Planned Parenthood and other radical organizations on the left would have you believe. They expect you believe the nonsensical claim that Re-publicans are waging a war on women if they dare oppose Planned Parenthood's anti-women agenda which will use ObamaCare as its vehicle to expand sexual license in the U.S., augment population control efforts, and open Planned Parenthood's coffers to receive a flood of taxpayer funds. In its implementation of ObamaCare, the U.S. Department of Health and Human Services has mandated that employers must purchase health insurance that would provide employees with free sterilizations and (cancer-causing) hormonal contraceptive steroids and abortifacients. That would include the birth control pill (oral contraceptives) and the injection, Depo Provera birth control. And the U.S. Supreme Court has agreed for the first time that forcing citizens to buy a product they don't want - in this case, health insurance - is a tax that the federal government may impose. More American women will die of cancer because of the ObamaCare mandate. Women and their unborn chil-dren will be sacrificed on Planned Parenthood's altar of greed! Like every other Big Lie that has been told throughout history when infamous political leaders and others have used propaganda to conceal their nefarious purposes, Planned Parenthood and its political allies have repeated their canned slogan, "Stop the war on women," ad nauseum in the hope it will ultimately stick. They would have you believe that women around the world are dying in the streets for lack of hormonal contraceptive steroids and induced abortions. They make it sound as if taking hormonal steroids and having abortions are as necessary to women's health as drinking water and breathing air! That's their idea of "reproductive health"! But here's the truth! As the promoters and sellers of the birth control pill and the injection, Depo Provera birth control, Planned Parenthood and its allies are clearly the ones waging a war on women! Five studies have strongly linked use of Depo Provera with a 1.5- to 2.3-fold increased breast cancer risk. (Other grave risks include increased susceptibility to HIV/AIDS and bone mineral density loss.) Christopher Li, Janet Daling and other researchers from the Fred Hutchinson Cancer Research Center in Seattle conducted the first "large scale U.S. study" on the link between use of Depo Provera and in-creased breast can-cer risk. They found a statistically significant 2.2-fold elevated risk of invasive breast cancer among women who had used Depo Provera recently for at least 12 months. Significantly, the authors noted that the 2003 Women's Health Initiative study demonstrated a 1.24-fold ele-vated risk among those who used estrogen plus progestin hormone replacement therapy (HRT). On the other hand, estrogen only HRT "had a nonstatistically significant reduced risk." Depo Provera contains only progestin. So why does it raise risk?

W H AT W E K N O W Managing Medication for Adults with AD/HDAD/HD is recognized as a neurobiological disorder that often persists from childhood into adulthood, resulting in about 4-5% of adults having AD/HD.1-5 Medication, which is an integral part of the multimodal treatment of AD/HD in children, is the cornerstone of treatment for adults.6-8 Although there is a significant amount of research in children, there is much less controlled research data on medication therapy in adults.4,9 Regarding the use of medication in the treatment of AD/HD, it has been said that "pills do not substitute for skills."45 This means that medication alone is not sufficient to help a person improve his or her problems in areas such as organization, time management, prioritizing, and using cognitive aids. However, medication levels the neurobiologic playing field, and allows adults with AD/HD to learn and develop the skills they need to succeed.

Margaret Williams 28th October 2013 [Plus UPDATE 21st April 2014 follows below] When a professional person – especially a doctor – has repeatedly been shown to be wrong in their professional judgment and, as a direct consequence, people have been harmed, that doctor should surely be held personally responsible and accountable: in such circumstances legitimate criticism should not be dismissed as an ad hominem (personal) attack.Following the award of the inaugural John Maddox Prize to psychiatrist Professor Sir Simon Wessely for his alleged "courage" in "standing up for science" and for promoting "sound science" about ME/CFS in the face of "hostility" in doing so, a letter published on 13th January 2013 by the Countess of Mar, Professor Malcolm Hooper and Dr William Weir in The Independent on Sunday was explicit that criticism of Wessely's hypothesis about ME/CFS is scientifically legitimate:"Scientific understanding always depends upon sound evidence….For scientific understanding to prevail, the extensive biomedical evidence-base of ME/CFS must now be recognised by all researchers in the field. The idea that ME/CFS is due to a dysfunctional psyche is a hypothesis without an evidence-base. The Maddox Prize was therefore awarded to the defender of an hypothesis with no evidence-base rather than to someone who was upholding true scientific inquiry. Personal attacks against Professor Sir Simon Wessely do not advance the cause, but it is scientifically legitimate to direct criticism at the hypothesis both he and Professor White (chief Principal Investigator of the MRC's PACE trial on ME/CFS) continue to espouse". It has been shown time and again that Professor Sir Simon's published assertions about disorders such as ME/CFS, fibromyalgia, Gulf War Syndrome, the Camelford drinking water poisoning, and interstitial cystitis are simply wrong. Merely stating so is likely to result in yet more claims by him of "harassment" and "attack" upon him but, in the words of Professor Martin Bland, one of the UK's leading medical statisticians, it is important that false information should not remain on the record to be quoted uncritically by others: "Potentially incorrect conclusions, based on faulty analysis, should not be allowed to remain in the literature to be cited uncritically by others" (Fatigue and psychological distress. BMJ: 19th February 2000:320:515-516). Wessely's "incorrect conclusions", however, remain in the literature to be cited uncritically by others and therefore may result in iatrogenic harm.ME/CFSFor over 25 years Wessely's dismissal and rejection of the biomedical evidence on ME has continued unabated, even though there is substantial evidence of on-going inflammation throughout the body; systems prominently affected are the central and autonomic nervous systems, the immune system and the cardiovascular, endocrine, gastro-intestinal and musculoskeletal systems.

Her symptoms were better with closed eyes, and diplopia was worse on left gaze with images one above the other. Her past medical history was significant for cerebellar astrocytoma at the age of 4 (which was treated with resection and location radiation therapy), seizure disorder, and ventriculoperitoneal shunt placement for treatment of hydrocephalus. She also had known hypertension. Her home medications included oxcarbazepine, verapamil, and oral contraceptive pills (OCP).

Making Education Easy This publication is a summary of a presentation by Dr Stephan Heres, Senior Psychiatrist at the Department of Psychiatry and Psychotherapy at the Centre for Disease Management at the Psychiatric Department, Technical University Munich, Germany. He spoke throughout New  Zealand in July/August 2012 on the attitudes of psychiatrists and patients towards depot (long-acting injectable) antipsychotic treatment of schizophrenia, the current efficacy evidence for depot antipsychotic use and the current outcomes of the OASE depot clinic in Munich. He acknowledges that the title of his presentation is a little provocative, but believes that the discussion of depot antipsychotic treatment as first-line therapy for schizophrenia is an important one.
